This treatment can be done at an out-of-pocket cost, or, prescribed by a licensed Physician or Nurse Practitioner. If you are coming in with a prescribed treatment, you will need to obtain a prescription for Botox® from your healthcare provider and have it filled at your pharmacy. Please bring your prescription Botox® with you to your appointment, as it is not supplied by the clinic.
